Advances in medical surgical technology have allowed procedures to now be performed with minimally invasive techniques using image-guided catheters. Surgeons create documentation summarizing these procedures, which must then be read and interpreted by health information professionals to support medical coding and billing, quality reporting, clinical research and subsequent care decisions for the patient. Risks of incorrect interpretation include medical errors, non-compliant coding and incorrect billing. The Actus® 6.0 product, developed by A-Life Medical, Inc., is the first software application available that integrates digital images of vascular anatomy with automated interpretation of physician documentation to provide real-time visualization and coding of catheter-based surgical procedures. With Actus® 6.0, the interpretation of the surgeon’s documentation is automated through the use of A-Life’s patented Natural Language Processing (NLP) software technology, LifeCode.

Sinusitis is inflammation of the sinus cavities that may be caused by infections, allergies or environmental factors and can cause significant pain and suffering for patients. According to the Centers for Disease Control, or CDC, approximately 13% of the U.S. adult population, or 37 million people, are affected by sinusitis. Acclarent is a company focused on developing and marketing devices for the Ear, Nose and Throat market. Acclarent markets a suite of Balloon Sinuplasty™ devices, which are flexible, minimally invasive tools that aid physicians in surgically treating patients suffering from chronic sinusitis. Balloon Sinuplasty™ instruments were used in 3,276 peripheral sinuses. There were no major adverse events attributable to balloon catheter use. The revision rate was 1.3% of sinuses with an average follow-up of 40.2 weeks. Sinus symptoms improved in 95.2% of patients

Nomination for their: HealthCare Partners Hospitalist Program at California Hospital Serving the Indigent & Medi-Cal Population

California Hospital Medical Center (CHMC) is a 316 bed Catholic non-profit hospital located in inner-city Los Angeles. The hospital is at financial risk because it serves a large uninsured and indigent population. CHMC is also a designated trauma center, triaging an average of 155 patients per day in the Emergency Department and caring for an average of 195 trauma cases per month. In collaboration, Healthcare Partners (HCP) and CHMC have developed a hospitalist program that has significantly improved the management of patient flow, quality of care patients receive, and expediency with which care is provided at CHMC. This arrangement turns a typically disjointed healthcare delivery system into a ‘community healthcare system” for optimal clinical outcomes at reduced costs. Heart failure patient readmission rates within 31 days were down 37% and were 5.68% better than standard for US hospitals.  Acute care readmission rates within 15 days were down 89% and were 5.59% better than standard for US hospitals.

Scientific literature shows that 40%-50% of birth traumas that occur in the US each year are potentially avoidable. PeriGen’s clinical decision support system specifically addresses the root causes behind 50% of preventable medical errors in OB: errors of communication and errors of cognition. The PeriGen system unites provider information on an OB-specific digital patient chart, which both improves communication between providers and eliminates errors which stem from inaccurate documentation. Provider access to complete, unified, well-documented, and clearly organized patient data not only reduces the likelihood of error, but results in improved charge capture and increased productivity, which demonstrably increase revenue. One client hospital was able to reduce the incidence of Erb’s Palsy to zero in less than five years, while yet another client used the PeriGen system to effectively eliminate the occurrence of elective cesarean delivery before 39 weeks gestation in less than one month.
